Strategic Directions
Primary Care Transformation
Enhancing access to equitable, team-based care through leadership, co-design, and data-driven planning. We are focused on expanding our capacity to serve unattached patients and ensuring seamless transitions.
Integrating Clinical Services and Supports
Coordinating care for people with complex needs, with a focus on mental health, chronic conditions, and cross-sector collaboration. This includes developing localized care pathways to reduce hospital readmissions.
Evolving the SOHT
Building a mature, resilient organization with refreshed governance, performance measurement, and sustainable operations. We will strengthen our partnerships and digital infrastructure to support our clinical goals.
